Who We Are

Hands to Heart Foundation is a 501(c)(3) nonprofit organization. Our nonmedical homecare services are provided through Hands to Heart Home Care (HCA.0001456), a separately registered homemaker-companion agency. DME coordination and community support services are provided through the foundation. Only nonmedical services are provided under our HCA license.

Homecare Services

Support with everyday activities and peace of mind for families.

  • Personal hygiene care (bathing, grooming, dressing).

  • Mobility assistance and transfers.

  • Medication reminders (nonmedical prompting only - not administering).

  • Meal prep and light housekeeping.

  • Companionship and safety checks.

  • Respite for family caregivers.

  • Transportation assistance to appointments

Durable Medical Equipment (DME)

  • Through our nonprofit foundation (separate from our HCA-licensed agency), we help identify the right equipment and coordinate fulfillment through reliable suppliers. DME coordination is not a homecare service and is provided separately.

  • Common items include:

  1. Mobility: walkers, wheelchairs, canes

  2. Bath safety: grab bars, shower chairs, commodes

  3. Home safety: bed rails, fall-prevention aids

  4. Comfort & support lift chairs, cushions, orthopedic supports

  5. Other essentials based on clinician recommendations

  • Do you accept insurance?

Coverage depends on the service and equipment. We’ll help you understand options and
available funding sources.

  • Do I need a prescription for DME?

Some items require a clinician’s order. We’ll coordinate with your provider when needed.

  • Where do you provide services?

We currently support clients in Connecticut. Contact us to confirm availability.

  • How soon can services start?

Timelines vary depending on the need and the availability of staffing and equipment. We
prioritize safety and urgent requests.
